Εδώ και 3 μήνες ξεκίνησαν οι συναντήσεις CPC(clinical-pathological conferences) στον χώρο της Παθολογικής Ανατομίας.Οι συναντήσεις διεξάγονται στο μικρό αμφιθέατρο της παθολογικής ανατομίας,ΟΧΙ στο αμφιθέατρο που γίνονται οι παραδόσεις της ΠΑΘΑΝ.Τρόπος διδασκαλίας είναι η παρουσίαση περιστατικών βγαλμένων μέσα από την κλινική πράξη και ακολουθεί συζήτηση επί των περιστατικών.

Προϋποθέσεις συμμετοχής:όρεξη για ενημέρωση πάνω σε cutting edge ζητήματα της ιατρικής.

Τα μαθήματα διοργανώνονται από την έδρα παθολογικής ανατομικής του ΕΚΠΑ και την θωρακοχειρουργική κλινική του πρώτου νοσοκομείου ΙΚΑ-πρώην Παπαδημητρίου.Η ιδιαιτερότητα:παρουσιάζονται πολλά περιστατικά που έχουν διερευνηθεί με VATS-video assisted thoracoscopy-μια ελάχιστα παρεμβατική τεχνική που δεν είναι ευρέως διαδεδομένη στην Ελλάδα.

Η πρώτη συνάντηση έγινε στις 23 Νοεμβρίου και χθες,14 Φεβρουαρίου,μέρα των ερωτευμένων,ο πνεύμονας είχε την τιμητική του.Θέματα που παρουσιάστηκαν χθες ήταν:

Νευροενδοκρινικός όγκος πνεύμονα

Ενδοβρογχικό άμάρτωμα

Πρωτοπαθής όγκος πνεύμονα με συνύπαρξη καρκινοειδούς(!).

Αμυλοείδωση τραχείας.

Μια άριστη εκπαιδευτική εμπειρία,αφού από το πρόγραμμα βλέπετε ότι ήταν μια γρήγορη και εύκολη επανάληψη για α)όγκοι πνεύμονα,καλοήθεις και κακοήθεις,σταδιοποίηση,πορεία,κλινική εικόνα και β)ενδοκρινικοί όγκοι,εντόπιση και διερεύνηση γ)παραπρωτεϊναιμίες και αμυλοείδωση-μια κοινή πάθηση στους ηλικιωμένους που όμως στα βιβλία δεν λαμβάνειι ιδιαίτερη μνεία.

Η επόμενη συνάντηση θα γίνει αρχές Απριλίου,οπότε και θα γίνει η σχετική ανακοίνωση.Η παρουσία στην συνάντηση μοριοδοτείται με 18 μόρια συνεχιζόμενης ιατρικής εκπαίδευσης,αναγνωρισμένα από τον UEMS.

The clinical pathological conference

In bygone years, the clinical pathological conference (CPC) was one of the most popular and effective methods for teaching clinical medicine. Although some schools of medicine continue to include the ‘CPC’ in the curriculum, many have moved to other formats to provide learning experiences for students, residents and fellows. Similarly, many clinical journals no longer use this format in their publications, although it is not clear that this is due to lack of reader acceptance or appreciation.

The notion of correlating clinical presentation, laboratory investigation and pathology (evident either morphologically or biochemically) is very appealing as a learning experience. Most of us find it easier to remember ‘the case’ than the print from the standard textbooks or the recent literature. Even when the case fell short of the ultimate goal of a pathological diagnosis, there were excellent opportunities to correlate the clinical presentation with what was not present. Given the advent of molecular diagnostic approaches, however, the CPC can now provide even more fundamental correlation between structure and function such that I suspect there will be fewer patients in whom the pathology remains undefined.

Why have we moved away from this classical method of clinical teaching? The reasons are varied. There has been a marked reduction in the rate of autopsies in our teaching hospitals. Our trainees spend less time in the teaching hospital environment, and more time in the community hospitals and outpatient clinics. There seems to be ever increasing workloads for all concerned such that there are fewer personal encounters between members of the clinical departments and those of the pathology department. Thus, the pathologist may not be aware of the unique presentation or complications in a particular patient and may therefore investigate only minimally. Finally, a great deal of planning and effort by both clinicians and pathologists is required to create a CPC that is informative enough to be the ‘case’ that will be remembered.

However, the CPC does remain a very effective educational tool that we would like to help perpetuate.
